Please read the following information carefully and inform the tattoo studio if you have any doubts about your suitability to be tattooed.
Tattooing and the healing process can come with risks and potential complications, as explained to you. You will be given aftercare instructions to help minimize the effects.
It is normal for tattooing to involve pain, redness and swelling. If these symptoms worsen during the healing process, it could be a sign of infection and you should consult a doctor.
Some complications may be related to your health situation. It is important for your safety that the tattoo artist is informed of this. Additional precautions may be necessary.
Tattoos are permanent and depending on the ink used, it is possible that they cannot be removed properly. If in doubt, treatment should be postponed and medical advice sought.
Do not get a tattoo:
• if you are pregnant or breastfeeding;
• if you are under the influence of alcohol or drugs;
• on a mole or scar left after the removal of a melanoma.
The following conditions or conditions may carry a higher risk of complications:
1) Skin:
• a) skin infection regardless of location and type of infection;
• b) allergies (indicate if you have an allergy, for example latex allergy, allergy to metals and preservatives);
• c) areas of skin that appear damaged or abnormal, including moles;
• d) any active skin diseases (e.g. eczema, psoriasis);
• e) areas of skin that have been subjected to plastic surgery or radiotherapy, including recent scars and keloids (as determined by medical opinion);
• f) a previously tattooed area that has not yet fully healed;
• g) an area where a tattoo has recently been removed by laser or other means if the wound has not yet healed. 2)
Common ailments:
• a) haemophilia or another clotting disorder;
• b) epilepsy, cardiovascular diseases;
• c) known allergies, e.g. latex allergy);
• d) diabetes;
• e) an autoimmune disease;
• f) immunosuppression and conditions with a compromised immune system;
• g) sarcoidosis.
If any of the above conditions or conditions apply to you and you still want to get a tattoo, consult your doctor first and ask for advice.

Tattoo Artist Statement:
“I confirm that tattooing is carried out under hygienic conditions using suitable sterile instruments and safe techniques and in accordance with EN 17169 or national requirements.”
“I do not administer any form of anesthesia for this treatment.”
“I confirm that the client has been offered a copy of this signed consent form for retrieval and that the client has been advised to retain the information.
